The IntraHealth NHS Immunisation Team will be visiting our school on Monday, 2nd December to administer the nasal flu vaccine to students across all year groups.
To ensure your child receives the appropriate vaccination, please complete the Online Consent Form
It is essential to fill out this form even if you do not wish to give consent. The form will also provide you with the option to choose between the nasal or injectable vaccine.
