The IntraHealth NHS Immunisation Team will be in school on Monday 23rd January to administer the nasal flu vaccine to students in Year 7, 8 and 9. If you haven’t done so already, please complete this online consent form. You need to complete this form even if you do not wish to give consent. It will also give you the option to choose the nasal or the injectable vaccine. Please complete this form as soon as possible, it will close for completion on 9th January at 11.59pm.
